Medical review policy

Clinical recommendations require an identifiable physician author, a named medical reviewer, review date, evidence cutoff, guideline version, geographic applicability and conflict disclosures. An empty review field means no review has taken place.

Treatment, dosing, safety, follow-up and standard-of-care changes are routed to review before being labelled practice-changing. Automated literature monitoring creates an editorial task; it never rewrites an approved recommendation.

Guides retain a change log and next-review date. Urgent safety signals receive priority; scheduled review alone is not a guarantee of current guidance. German and Italian clinical language require separate terminology review.
